Before your first session, please complete the form corresponding to your selected service. These forms help us prepare for your visit and ensure that your care is responsive, informed, and centered on your needs.
Just so you know, all responses are confidential. If you’re unsure which form to complete, start with our general consultation form or contact us at (615) 212-8759 or info@curatemedinsights.com.
Once we confirm your service and session length, I’ll send a payment link and receipt. You can pay via Zelle/Stripe/FSA card and we’ll schedule you fully. Let me know if you plan to use insurance or a Flex plan so I can send documentation.
